This form is for parents to inform us if your child (who attends Dell Primary) has received a positive test result for COVID-19. If you receive the test result in school hours, you should phone the school office on 01502 565956.
The safety of our staff and children is paramount, we therefore need to know immediately of any positive cases in school.
If your child has received a positive test result for COVID-19, the whole household MUST stay at home for 14 days from onset of symptoms.
